Sharing your story consent form

Thank you for agreeing to share your experience with SeeSaw. By telling your story, you help others understand the real impact of bereavement support and why our work matters. Your words can help families feel less alone and encourage supporters to continue making this work possible.

Your story may be shared through our newsletter, website, fundraising materials, appeals, and social media. You can choose exactly where your story is used, and you may remain anonymous if you prefer.

The space provided for each question is not an indication of how much you should write. Please share only what you feel comfortable with—in as much or as little detail as you wish. If other family members would like to share their experience, they are welcome to contribute to this form or complete their own.

By completing this form, you give permission for SeeSaw to share your story with supporters, donors, and members of the public, in line with the choices you make below.
